Start together
Guidedly begins during discharge, helping reinforce education and prepare patients and caregivers for what comes next.
About Guidedly
Guidedly was built around a simple belief: no one should feel alone trying to understand what comes next after a hospital stay.
We work alongside care teams at discharge and stay connected with patients and caregivers through the critical 30 days that follow.
Why Guidedly Exists
Guidedly was shaped by a deeply personal experience.
When my grandfather, George, was hospitalized, my family was trying to understand a complicated care journey while carrying fear and uncertainty. Information passed from one family member to another, and questions surfaced after conversations had ended. At times, understanding what was happening—and what would come next—felt like a game of telephone.
After he left the hospital, that uncertainty became harder. His condition deteriorated quickly. Looking back, some of the gaps we experienced may have been easier to recognize with clearer guidance, stronger follow-through, and a better understanding of where to turn with questions or concerns.
That experience stayed with me. Over time, I heard versions of the same story from others—families overwhelmed by instructions, patients unsure what to do next, and people without a strong support system managing appointments, medications, symptoms, and follow-up care on their own.
Guidedly was created to help change that experience.
Because no one should feel alone in the transition from hospital to what comes next.

A Familiar Gap
Discharge can ask patients and caregivers to absorb a lot at once—medications, appointments, instructions, warning signs, follow-up care, and changes to daily routines.
Questions often surface later, when the care team is no longer right in front of them. For people without strong family support, navigating those next steps can be even harder.
Our Approach
Guidedly is designed to work alongside existing clinical workflows—not replace them.
Guidedly begins during discharge, helping reinforce education and prepare patients and caregivers for what comes next.
Translate the care plan into understandable next steps, answer recurring questions, and help people know where to turn.
Continue checking in through day 30, helping surface barriers or concerns and connecting patients to appropriate support when needed.
Built for Both Sides of the Transition
Clearer next steps, answers to common questions, continued guidance, and a place to turn when something doesn’t feel right or a barrier gets in the way.
An added layer of transition support that reinforces the care plan, extends education beyond the discharge conversation, surfaces barriers earlier, and gives care teams greater visibility into what happens next.
